Patient Overview
This patient presented at our clinic with advanced male pattern hair loss, characterised by a significantly receded hairline and a fully depleted crown area.
He had previously undergone Scalp Micropigmentation (SMP) across the scalp, which provided a visual illusion of density but did not restore natural hair growth. His goal was to achieve a real, natural-looking hairline and visible density across the crown, while maintaining a clean and undetectable donor area.
Treatment Plan: Two-Day FUE Hair Transplant
Following a detailed consultation and donor assessment, we recommended a two-day Follicular Unit Extraction (FUE) hair transplant to maximise coverage and density.
Day 1: Crown Reconstruction
The first day focused entirely on the crown (vertex), which was fully depleted.
- A new crown was designed from scratch, including a natural whorl pattern to mimic natural hair growth direction
- Grafts were strategically placed to create balanced density and coverage
- Existing SMP was taken into account to enhance the visual outcome
Day 2: Hairline Restoration
The second day focused on the hairline and frontal area:
- The hairline was lowered and reshaped to better suit the patientβs facial proportions
- A natural, irregular hairline design was created to avoid a harsh or artificial appearance
- Grafts were placed into and around existing hair to ensure a seamless blend and improved density
Combining Hair Transplant with SMP
One of the key aspects of this case was working alongside previous SMP treatment.
Rather than removing or disregarding it, we used it to our advantage by:
- Layering transplanted grafts over SMP to enhance the perception of density
- Creating a natural transition between real hair and scalp shading
- Achieving a result that looks fuller even at earlier stages of growth
This combined approach is particularly effective in patients with extensive hair loss.
Results at 6 Months Post-Op
At just 6 months post-op, the patient has already achieved excellent early results.
- Noticeable growth across the hairline and crown
- A clearly defined, natural-looking hairline

- Strong overall transformation despite being mid-growth phase
It is important to note that 6 months is only the halfway stage in hair transplant recovery.
Further thickening, density, and maturation will continue over the next 6β12 months, particularly in the crown area which typically takes longer to develop.
Donor Area Healing & Recovery
An equally important aspect of any successful hair transplant is the condition of the donor area.
In this case, the donor area shows:
- Even and well-distributed extraction
- No visible patchiness or overharvesting
